A Release Form keeps the current semen owner as the legal owner of the semen and only one semen owner needs to sign off on a release. They are releasing the semen for the desired breeding as listed on the release to a recipient/bitch owner. When a litter is produced, the semen owner will have to sign off on the litter registration, not the recipient/bitch owner.
This form is valid for 90 days upon receival.

To be used to transfer ownership of frozen canine semen stored at ICSB.
A Transfer of Ownership transfers full ownership of the semen. All current owners of the semen must sign off on a transfer. This makes the recipient the legal owner of the semen and they will be the one to sign off on the litter registration when a litter is produced from the semen. Each Transfer (per stud) incurs a $65 transfer fee. The new semen owner will then need to complete a Release Form to send or use the semen.

A guide to the proper use and packing of the ICSB Puppy Pak. This kit is used to ship collected semen fresh-chilled from a stud owner to a bitch owner/veterinarian.
